About Us

The Maple Family Health Team (Maple) is a primary health care organization that brings together family doctors, nurse practitioners, registered nurses, social workers, dietitians, and other professionals to provide and coordinate the best possible primary care for patients. 

Maple is a not-for-profit Corporation, managed by a Board of Directors, and has six clinics in the Kingston area.

What does it mean to be a patient of the Maple Family Health Team?

Family Health Teams are patient centered and you will have access to a number of primary health care services from a variety of team members, including your family doctor. 

Team-Based Care

If you see another Maple health care provider for treatment, your doctor will receive documentation about the nature of your problem so that they will always be up-to-date about your medical condition.

 

After Hours and Weekends

You will have access to a Maple doctor during extended evening and weekend hours for urgent problems.

Professional Resources

You will have access to other health care professionals within Maple: nurse practitioners, nurses, mental health counsellors, dietitians and pharmacists.

Telephone Health Advisory Service

If you need after-hours care, you can phone the Telephone Health Advisory Service at 1-866-553-7205 where a registered nurse will provide advice about your urgent health care concerns.
