Programs and Services
Maple Family Health Team

At the Maple Family Health Team (FHT), we offer a variety of services and programs to meet the needs of our patients. In addition to the care that you receive from your family doctor, the Maple FHT also has other health care providers available to support your needs. These additional programs and services are listed below and are often provided right in your doctor’s office. These programs support your health, by either helping to manage an existing disease or assisting in preventing future disease. 

General Medical Care

The family doctors of the Maple Family Health Organization provide the majority of your medical primary care.  The Maple Family Health Team is complementing these services with nurse practitioners and other health care providers. Our Nurse Practitioners work in collaboration with Physicians and Registered Nurses to care for patients in many different ways. 

 
Some of your care will be provided by other health care providers within the team either by referral or as part of general medical care. Each of the programs listed describes how other health care providers support your care.
Chronic Obstructive Pulmonary Disease Management

The COPD Management program is in place to improve COPD patients’ health and to assist patients in optimizing their health while managing the disease throughout its progression. Speak to your health care provider for more details.

Diabetes Management

The Maple Family Health Team (FHT) “Diabetes Management” program has been developed to improve the patient’s health and to assist patients in having a controlled and managed disease. The Maple FHT nurses are trained in the diabetes Clinical Practice Guidelines and work collaboratively with your family physician or Nurse Practitioner. The program is developed around regularly scheduled visits where nurses review and record the recommended aspects of a diabetes patient’s health. These visits may also include patient education in areas such as: glucose control, meter use, hypoglycemia, taking medications, healthy eating, foot care, and more. This regular monitoring, in conjunction with the care provided by your family physician, supports patients in managing their diabetes. 

Mental Health Services

Many people will experience a mental health-related problem or mental illness at some point in their lives. The Maple Family Health Team (FHT) has a team of Mental Health service providers to assist people at these times. The Mental Health team consists of a psychiatrist, psychologist and mental health counsellors. Family doctors and the Mental Health team work together to help people with mental health related problems.  These Mental Health Services are available to people of all ages, including children, teens, adults and seniors. The Maple FHT continues to build links to other Mental Health service providers in the community to provide you the best care available. 

Nutrition Services

CLINICAL NUTRITION SERVICES

Registered Dietitians (RD's) are experts in the area of nutrition as well as interpersonal and counselling skills.  They are uniquely trained to provide individualized, evidence-based nutrition therapy to patients.  The long term goals are to improve health, increase longevity and enhance enjoyment of life.    
 
Weight Management

The Weight Management program is a non-dieting, healthy lifestyle approach designed to help obese and overweight patients improve their eating habits and find ways to increase activity levels. This group program is offered so that participants might:

·         Achieve a healthier weight
·         Reduce their health risk
·         Feel better both physically and mentally
 
The group program is led by a Registered Dietitian, with components facilitated by a certified personal trainer and a Mental Health Counsellor.  Group participants must have previously attended the introduction to Weight Management session.  
Foot Care Clinic

The Maple Family Health Team provides foot care services for patients.  Speak to your health care provider for more details. 

Pharmacist Services

The Maple Family Health Team has a pharmacist on staff available for individual patient counselling, either face-to-face or over the phone. Ask your health care provider for details. 

Immunization

Regular immunization is an ongoing part of patient care at the Maple Family Health Team. Immunization against disease helps in the prevention of disease and most immunizations can be performed in your physician’s office. You or your family can also get immunized at your local Public Health Unit. Information regarding immunizations is available through the Public Health Unit.

When:  Nov 5th,  Nov 12th & Nov. 19th (Saturdays)
            9:00 am-1:00 pm
 
Where: Located at the Maple Family Health Team Central Site
             1036 Princess St. at the Kingston Centre next to Pharma Plus
 
Who:   Open to the patients of the Maple Family Health Team that are age 6 months and older.
 
Note that vaccines will be given in an open area that offers little privacy. Please wear a short sleeved shirt to allow easy access to the injection site.
 
 
Please bring your Ontario Health Card.

You should not get a flu shot if....
*  You have had a severe reaction to the flu shot in the past.
*  You have a know allergy to eggs or thimerosal.
*  You are ill on the day you plan to get the flu shot.

What are the risks and how might I feel after my shot?
The vaccine, like any medication, is capable of causing side effects, which can be either mild or, occasionally, severe.  The risk of the vaccine causing serious harm is extremely small.  Most people who get the vaccine have either no side effects or mild side effects.  A Ministry of Health "Influenza Vaccine Fact Sheet" is available on the websites listed below.

Smoking Cessation

The Smoking Cessation program provides a systematic approach to assisting patients when they are ready to quit smoking. As part of routine patient care, nurses may ask about your tobacco use and your readiness to quit.   The Maple Family Health Team nurses have been trained in identifying the ways in which your health care team and other agencies can assist you with your “quit” goal. If you are a tobacco user and are ready to quit, ask your health care provider how they might assist you.

Healthy Hearts

Healthy Hearts

What is the Healthy Hearts program?
A 6 week group program consisting of a 2 hour session each week. It is co-led by a Registered Dietitian and a Mental Health Counsellor, with components facilitated by a Physician, Pharmacist and Occupational Therapist.  These Health Professionals will help you start taking steps toward CHANGING your lifestyle to improve your health and lower your risk for Cardiovascular Disease.   
 
Who is this program for?
People who are at an increased risk for heart disease and have at least three of the following health issues:
·         High Cholesterol
·         High Blood Pressure
·         Diabetes or Pre-Diabetes
·         Tobacco use
·         Family history of heart attack, stroke or coronary artery disease
·         Obesity
·         Inactive lifestyle
·         Unhealthy diet
 
How can I participate in the program?
By speaking to your health care provider who will begin the process if you are eligible. Participating in this program is a two-step process (click "Program Details" at the bottom of this page for more information).
  
How do I know if this program is right for me?
·         I have three or more risk factors for heart disease.
·         I want to learn how I can lower my risk for heart disease.
·         I am ready to start making changes to improve my health.
·         I have tried unsuccessfully to make changes on my own.
