Maple Family Health Team

Maple Family Health Team is comprised of 26 physicians in six clinics in Kingston.  Nurse practitioners, registered nurses and other health care professionals work together with our physicians to provide you with effective, timely and efficient care.  For more information about our team, please go to 'contact us'.

What is a Family Health Team?

A Family Health Team is a new way of providing health care to you and your family in Ontario.  It is a team approach that brings together family doctors with other health care providers in order to coordinate the best possible care for patients.  Everyone on the team works together to meet your needs.  Their focus is on keeping you and your family healthy, not just treating you when you are sick.

When you enrol as a patient with a Family Health Team, you are taking a step to ensure that you always have ready access to health care from a family doctor and a team of health care providers.



What does it mean to be a patient of the Maple Family Health Team?

Family Health Teams are patient centered and you will have access to a number of primary health care services from a variety of team members, including your family doctor. For example:

What are some of the programs and services the Maple Family Health Team provides?

The Maple Family Health Team (FHT) provides programs and services to augement the care provided to you by your family physician.  See "Progams and Services" for a full discription of the programs that are currently being offered at the Maple FHT.

Will I still get to see my doctor if he or she is part of the Maple Family Health Team?

You will continue to see your family doctor during regular office hours and scheduled after hours if he or she is available.  If your doctor is unavailable, you will be able to see another doctor  or a Nurse Practitioner on the Family Health Team.  Your family doctor will be informed about your visit, including the nature of the health concern and the advice you received.  This helps ensure continuity of care.
