Where To Get Your Flu Shot:

1.  Doctor’s Office:  Patients aged 65 years and older can contact their family doctor’s office to book an appointment.  Offices may offer the regular dose or the high dose if available.

2.  November 12th Clinic:  Maple FHT will be running a flu shot clinic, by appointment only, on Thursday, November 12th at the Memorial Centre.  This clinic is only open to patients of the Maple Family Health Team.  There will be no high dose vaccine at this clinic.  See further information below.

3.  Pharmacies:  Pharmacies are also providing flu vaccinations and in addition to the regular dose, may also have high dose available.  Please call your pharmacy to book an appointment.

 

November 12th Flu Shot Clinic

Please Read This Before Booking Your Appointment:

  • You must be a patient of Maple FHT to book an appointment.  If you are booking for family members, they must each be a patient of Maple FHT and have their own appointment.
  • 12 hours prior to your appointment, you must take and pass the on-line COVID-19 screening assessment provided here
  • On the day of your appointment, you will be screened at the door for COVID-19 before being allowed entry into the facility.
  • Patients scheduled for an appointment are to follow the Flu Clinic Guidelines below.  Click here if you would like to print a copy of these instructions.  
  • The regular influenza vaccine will be available.  No high-dose vaccine will be available for patients over 65 years.

Flu Clinic Guidelines

  • Clinic Location. The flu clinic will be held at the Memorial Centre, 303 York Street, Kingston on Thursday, November 12.
  • Arrive at your scheduled appointment time.  This assists with maintaining Public Health requirements to reduce crowds and support physical distancing.   
  • Arrive with a mask and wear it.  Only patients wearing a mask will be allowed entry.
  • Wear accessible clothing. Immunizations will be given in a public space that offers no privacy. Please wear a t-shirt under your coat.
  • Bring your health card.
  • Attend only if you are feeling well and have no symptoms of a cold, flu or covid-19.  If you do not feel well, stay home.  If you do not pass the screening at the door, you will not be allowed to enter the clinic. 
  • Be respectful to the staff and volunteers at the clinic.
