Diabetes Management
The Maple “Diabetes Management” program has been developed to improve the patient’s health and to assist patients in having a controlled and managed disease. Maple nurses are trained in the diabetes Clinical Practice Guidelines and work collaboratively with your Family Physician or Nurse Practitioner.
The program is developed around regularly scheduled visits where nurses review and record the recommended aspects of a diabetes patient’s health. These visits may also include patient education in areas such as: glucose control, meter use, hypoglycemia, taking medications, healthy eating, foot care, and more. This regular monitoring, in conjunction with the care provided by your family physician, supports patients in managing their diabetes.
Maple also offers group education programs to assist in preventing and managing your diabetes.
